The bus interface functions txctl and rxctl may be used while the device
can not be accessed, eg. upon driver .remove() callback. This patch will
immediately return -EIO when this is the case which speeds up the module
unload.

The fields of struct acpi_device are only known when CONFIG_ACPI is
defined. Fix this by using a helper function. This will resolve the
issue found in linux-next:

ACPI will manage WiFi chip's power state during suspend/resume
process on some tablet platforms(such as ASUS T100TA). This is
not supported by brcmfmac driver now, and the context of WiFi
chip will be damaged after resume. This patch informs ACPI not
to manage WiFi chip's power state.

commit 330b4e4be937 ("brcmfmac: Add wowl support for SDIO devices.")
changed the behaviour by removing the MMC_PM_KEEP_POWER flag for
non-wowl scenario, which needs to be restored. Another necessary
change is to mark the card as being non-removable. With this in place
the suspend resume test passes successfully doing:
# echo devices > /sys/power/pm_test
# echo mem > /sys/power/state
Note that power may still be switched off when system is going
in S3 state.

Several host controllers supporting runtime-pm are causing issues
with our sdio wireless cards because they disable the sdio interrupt
upon going into runtime suspend. This patch avoids that by doing
a pm_runtime_forbid() call during the probe. Tested with Sony Vaio
Duo 13 which uses sdhci-acpi host controller.

On removal of SDIO card both functions of card will be getting
a remove call. When the first is hanging in ctrl frame xmit then
the second will cause oops. This patch fixes the xmit ctrl
handling in case of serious errors and also limits the handling
for remove to function 1 only.

Borrowed the idea of the PM freezer to make sdio suspend wait for
watchdog and DPC thread to freeze at a safe point in their thread
routine. The suspend takes 20-25 msec.

Each bus driver is maintaing an exported bus state indicating
if upper layers can or cannot send data. SDIO is using this state
also for more private states. This makes handling the states and
state changes complex. This patch minimises the exposed states
and makes SDIO keep track of an internal state where necessary.

Before going in suspend state the watchdog thread needs to put the
device in bus sleep state, which assures it can go in deep-sleep
state during D3 state.

When the system is resumed a deadlock can occur when DPC gets
entered before resume is complete. This patch fixes this by
properly checking the suspend state outside the claim_host code
block.

The driver looks for pdata->oob_irq_supported to find out if wowl can be
supported. However, not all platforms populate pdata in which case we crash
the kernel because of NULL pointer dereference.

It has taken me a long long time to get the OOB interrupt working on the
AP6210 sdio wifi/bt module found on various Allwinner A20 boards. In the
end I found these magic register pokes in the cubietruck kernel tree:

brcmfmac devices can use an out-of-band interrupt on a GPIO line.
Currently this is specified using platform data. Add support for
specifying out-of-band interrupt via device tree.

In the function brcmf_sdiod_request_data() an error message is logged,
but the calling function retries it. This patch will only log an error
message when retry limit is reached. The low-level error is still
logged by a SDIO debug message.

The suspend callback first sets the suspend flag used in the driver
but after that the actual suspend is done, which may fail. Reset the
flag upon suspend failure.

Instead of allocating a scatter-gather table for every transmit
reuse a pre-allocated table. The transmit path will be faster by
taking out this allocation.

the function brcmf_sdiod_ramrw is supposed to be able to send
large blobs of data. However inside the loop the skb->len field
did not correctly get reset each round. As a result only small
blobs could be sent. This patch fixes this problem.

When removing the card the driver still tries to access registers
in the device. This patch adds another state for the bus that
indicates the device is no longer reachable. This avoids errors
accessing it while cleaning up the driver.

The struct brcmf_sdio_dev contains array of sdio functions that
are used in the driver. However, during probe func[0] entry was
assigned to the function 1 reference. This was corrected upon
doing the actual I/O access. This patch makes it more clear by
creating the func[0] entry properly and use it as is during
I/O access.

A new protocol layer msgbuf will be added in the future. This
change makes it possible to select the desired layer by the
bus driver. USB and SDIO will select BCDC. At the moment
nothing is being done with this information.

The SDIO send and receive functions in bcmsdh.c are always called
with the same parameters. For the driver there is no use-case
to call them otherwise so remove those parameters from function
prototypes.

Functions in bcmsdh.c that are called with struct brcmf_sdio_dev
instance are renamed consistently with brcmf_sdiod_ prefix. Also
removing brcmf_sdioh_attach/detach() functions and merge it with
brcmf_sdiod_probe/remove().

The chip recognition requires a base address that was provided
to it during the probe. However, the address is a fixed define
value so it is unnecessary to pass through the probe functions.

Instead of catching CCCR_IOEx register in F0 write access to
determine whether F2 state needs to change do it with direct
call to sdio_[enable/disable]_func().

During the SDIO probe the func_cis_ptr attribute in struct brcmf_sdio_dev
is being determined, but it is never used after that. Removing it also
obsoletes two functions in bcmsdh.c.

With the merge of bcmsdh.c and bcmsdh_mmc.c several function prototypes
are no longer necessary and can be removed. Consequence is reordering
some of the functions.
