Add SPDX license identifiers to all Make/Kconfig files which:
- Have no license information of any form
These files fall under the project license, GPL v2 only. The resulting SPDX
license identifier is:
GPL-2.0-only

The only difference between the hotplug implementation for Realview
and Versatile Express are the bit in the auxiliary control register
to disable coherency. Combine the two implentations accounting for
that difference.
Rename the functions to try to discourage cargo-cult copying of this
code.

This deletes the realview boardfiles, consolidates a bit
around the Kconfig options and leaves the mach-realview
directory nice and tidy, with all boards migrated over to
Device Tree.

This reduces the Kconfig for the RealView by assuming we are
always booting from the device tree, and removing all the uses
of CONFIG_REALVIEW_DT and replacing with CONFIG_ARCH_REALVIEW.
Further:
- Drop REALVIEW_HIGH_PHYS_OFFSET: we don't use this with device
tree.
- Drop the REALVIEW_EB_ARM11MP_REVB option: we now handle this
by simply using another device tree.
- Drop the PB1176 secure flash option: this is defined in the
PB1176 device tree but marked as "disabled", so users who
want to use it can simply enable it in the device tree and
go hacking around.

The device tree boot is now feature-complete and fully working,
so remove all the RealView board files. Move the remaining
realview_cpu_die() function frome "core.h" to the new "hotplug.h"
and leave just a minimal device tree core.

Three platforms used to have header files in include/mach that
are now all gone, but the removed directories are still being
included, which leads to -Wmissing-include-dirs warnings.
This removes the extra -I flags.

As it happens, two obj-$(CONFIG_FOO) += foo.o variables are
above obj-y := core.o, which doesn't work: the += directives
need to add something to the build and doesn't work if obj-y
is not set first, so move the obj-y to be on top and everything
builds nicely again.

The platsmp-dt.c file does not build when CONFIG_SMP is disabled:
platsmp-dt.c:84:2: error: unknown field 'smp_prepare_cpus' specified in initializer
This changes the Makefile to build it conditionally on CONFIG_SMP.
The legacy platsmp.c file is only used for ATAGS based builds, so
we can move it into the CONFIG_ATAGS conditional.

This adds an SMP boot method for the ARM RealView reference
designs. We also select HAVE_SMP by default and make it use
SMP_ON_UP so we only need to support one single kernel across
the RealView reference designs when using DT.
The RealViews need to have the SCU (Snoop Control Unit)
activated on boot, and this is now done by looking up its
address from the device tree and initializing it and counting
the available cores.
The RealViews boot by using a magic address register in the
system controller (SYS_FLAGS) to store the boot address,
the ROM will then read this register to the PC when the CPUs
are taken out of WFI. This code uses a handle to the syscon
regmap to access this register.

This implements basic device tree boot support for the RealView
platforms, with a basic device tree for ARM PB1176 as an example.
The implementation is done with a new DT-specific board file
using only pre-existing bindings for the basic IRQ, timer and
serial port drivers. A new compatible type is added to the GIC
for the ARM1176.
This implementation uses the MFD syscon handle from day one to
access the system controller registers, and register the devices
using the SoC bus.

This is a RealView platform supporting core tiles with ARM11MPCore,
Cortex-A8 or Cortex-A9 (multicore) processors. It has support for MMC,
CompactFlash, PCI-E.

This patch adds dummy local timers for each CPU so that the board clock
device is used to broadcast events to the other CPUs. The patch also
adds the declaration for the dummy_timer_setup function (the equivalent
of local_timer_setup when CONFIG_LOCAL_TIMERS is not set).
Due to the way clockevents work, the dummy timer on the first CPU has to
be registered before the board timer.
