As it was a synonym for (CONFIG_ACPI && CONFIG_X86),
the ifdefs for it were more clutter than they were worth.
For ia64, just add a few stubs in anticipation of future
S3 or S4 support.

EFER varies like %cr4 depending on the cpu capabilities, and which cpu
capabilities we want to make use of. So save/restore it make certain
we have the same EFER value when we are done.

For consistency and to have only a single place of definition, replace
set_debug() uses with set_debugreg(), and eliminate the definition of
thj former.

This patch fixes register saving so that each register is only saved once,
and adds missing saving of %cr8 on x86-64. Some reordering so that
save/restore is more logical/safer (segment registers should be restored
after gdt).

Initial git repository build. I'm not bothering with the full history,
even though we have it. We can create a separate "historical" git
archive of that later if we want to, and in the meantime it's about
3.2GB when imported into git - space that would just make the early
git days unnecessarily complicated, when we don't have a lot of good
infrastructure for it.
Let it rip!
