Aborts all requeued commands prior to killing the request_queue. For
commands that time out on a dying request queue, set the "Do Not Retry"
bit on the command status so the command cannot be requeued. Finanally, if
the driver is requested to abort a command it did not start, do nothing.

This protects admin queue access on shutdown. When the controller is
disabled, the queue is frozen to prevent new entry, and unfrozen on
resume, and fixes cq_vector signedness to not suspend a queue twice.
Since unfreezing the queue makes it available for commands, it requires
the queue be initialized, so this moves this part after that.
Special handling is done when the device is unresponsive during
shutdown. This can be optimized to not require subsequent commands to
timeout, but saving that fix for later.
This patch also removes the kill signals in this path that were left-over
artifacts from the blk-mq conversion and no longer necessary.

Since there is no gendisk associated with the admin queue, the driver
needs to hold a reference to it until all open references to the
controller are closed.
This also combines queue cleanup with freeing the tag set since these
should not be separate.

Once the nvme callback is set for a request, the driver can start it
and make it available for timeout handling. For timed out commands on a
device that is not initialized, this fixes potential deadlocks that can
occur on startup and shutdown when a device is unresponsive since they
can now be cancelled.
Asynchronous requests do not have any expected timeout, so these are
using the new "REQ_NO_TIMEOUT" request flags.

"After stopping the full x86/apic branch, I took some time to go
through the first block of patches again, which are mostly cleanups
and preparatory work for the irqdomain conversion and ioapic hotplug
support.
Unfortunaly one of the real problematic commits was right at the
beginning, so I rebased this portion of the pending patches without
the offenders.
It would be great to get this into 3.19. That makes reworking the
problematic parts simpler. The usual tip testing did not unearth any
issues and it is fully bisectible now.
I'm pretty confident that this wont affect the calmness of the xmas
season.
Changes:
- Split the convoluted io_apic.c code into domain specific parts
(vector, ioapic, msi, htirq)
- Introduce proper helper functions to retrieve irq specific data
instead of open coded dereferencing of pointers

was smuggeled in via the thermal tree. People have tried to hack
around it in the nohz core code, but it just violates all rightful

The powerclamp trainwreck will still work, it just wont get the

commit 4dbd27711cd9 "tick: export nohz tick idle symbols for module
use" was merged via the thermal tree without an explicit ack from the
relevant maintainers.
The exports are abused by the intel powerclamp driver which implements
a fake idle state from a sched FIFO task. This causes all kinds of
wreckage in the NOHZ core code which rightfully assumes that
tick_nohz_idle_enter/exit() are only called from the idle task itself.
Recent changes in the NOHZ core lead to a failure of the powerclamp
driver and now people try to hack completely broken and backwards
workarounds into the NOHZ core code. This is completely unacceptable
and just papers over the real problem. There are way more subtle
issues lurking around the corner.
The real solution is to fix the powerclamp driver by rewriting it with
a sane concept, but that's beyond the scope of this.
So the only solution for now is to remove the calls into the core NOHZ
code from the powerclamp trainwreck along with the exports.

Bhargava).
- Fix two regressions introduced by recent commits in the OPP library

- Continue to replace CONFIG_PM_RUNTIME with CONFIG_PM throughout the
tree (or drop it where that can be done) in order to make it
possible to eliminate CONFIG_PM_RUNTIME (Rafael J Wysocki, Ulf
Hansson, Ludovic Desroches).
There will be one more "CONFIG_PM_RUNTIME removal" batch after this
one, because some new uses of it have been introduced during the
current merge window, but that should be sufficient to finally get
rid of it.
- Make the ACPI EC driver more robust against race conditions related
to GPE handler installation failures (Lv Zheng).
- Prevent the ACPI device PM core code from attempting to disable
GPEs that it has not enabled which confuses ACPICA and makes it
report errors unnecessarily (Rafael J Wysocki).
- Add a "force" command line switch to the intel_pstate driver to
make it possible to override the blacklisting of some systems in
that driver if needed (Ethan Zhao).
- Improve intel_pstate code documentation and add a MAINTAINERS entry
for it (Kristen Carlson Accardi).
- Make the ACPI fan driver create cooling device interfaces witn
names that reflect the IDs of the ACPI device objects they are
associated with, except for "generic" ACPI fans (PNP ID "PNP0C0B").
That's necessary for user space thermal management tools to be able
to connect the fans with the parts of the system they are supposed

This patch changes core_scsi3_pro_release() logic to allow an
existing AllRegistrants type reservation to be re-reserved by
any registered I_T nexus.
This addresses a issue where AllRegistrants type RESERVE was
receiving RESERVATION_CONFLICT status if dev_pr_res_holder did
not match the same I_T nexus, instead of just returning GOOD
status following spc4r34 Section 5.9.9:
"If the device server receives a PERSISTENT RESERVE OUT command
with RESERVE service action where the TYPE field and the SCOPE
field contain the same values as the existing type and scope

An earlier commit to resolve an issue with encapsulation offloads missed
setting a bit in the outer netdev features flag. This results in loss of TSO
feature on a VxLAN interface.

Commit bc96f648df1bbc2729abbb84513cf4f64273a1f1 (xen-netback: make
feature-rx-notify mandatory) incorrectly assumed that there were no
frontends in use that did not support this feature. But the frontend
driver in MiniOS does not and since this is used by (qemu) stubdoms,
these stopped working.
Netback sort of works as-is in this mode except:
- If there are no Rx requests and the internal Rx queue fills, only
the drain timeout will wake the thread. The default drain timeout
of 10 s would give unacceptable pauses.
- If an Rx stall was detected and the internal Rx queue is drained,
then the Rx thread would never wake.
Handle these two cases (when feature-rx-notify is disabled) by:
- Reducing the drain timeout to 30 ms.
- Disabling Rx stall detection.

CEPH_OSD_OP_DELETE is not an extent op, stop treating it as such. This
sneaked in with discard patches - it's one of the three osd ops (the
other two are CEPH_OSD_OP_TRUNCATE and CEPH_OSD_OP_ZERO) that discard
is implemented with.

The functions ceph_put_snap_context() and iput() test whether their
argument is NULL and then return immediately. Thus the test around the
call is not needed.
This issue was detected by using the Coccinelle software.

These drivers haven't been tested in a long, long time. The hardware is
ancient and hopelessly obsolete. These drivers also need to be converted
to newer media frameworks but due to the lack of hardware that's going
to be impossible. In addition, cheaper and vastly better hardware is
available today.
So these drivers are a prime candidate for removal. If someone is
interested in working on these drivers to prevent their removal, then
please contact the linux-media mailinglist.
Let's be honest, the age of parallel port webcams and ISA video capture
boards is really gone.

This driver hasn't been tested in a long, long time. The company that made
this chip has gone bust many years ago and hardware using this chip is next
to impossible to find.
This driver needs to be converted to newer media frameworks but due to the
lack of hardware that's going to be impossible. Since cheap alternatives are
easily available, there is little point in keeping this driver alive.
In other words, this driver is a prime candidate for removal. If someone is
interested in working on this driver to prevent its removal, then please
contact the linux-media mailinglist.

The start_streaming op is responsible for starting the video dma,
so it shouldn't be called anymore from the buf_queue op.
Unfortunately, this call to start_video_dma() was added to the
start_streaming op, but was forgotten to be removed from the
buf_queue op, which is where it used to be before the vb2 conversion.
Calling this function twice causes very hard to find errors: sometimes
it works, sometimes it doesn't. It took me a whole friggin' day
to track this down, and in the end it was just luck that my eye suddenly
triggered on that line.

The cx88 vb2 conversion and the vb2 dma_sg improvements were developed separately and
were merged separately. Unfortunately, the patch updating drivers to the dma_sg
improvements didn't take the updated cx88 driver into account. Basically two ships
passing in the night, unaware of one another even though both ships have the same
owner, i.e. me :-)
