Convert docs to ReST and add them to the arch-specific
book.
The conversion here was trivial, as almost every file there
was already using an elegant format close to ReST standard.
The changes were mostly to mark literal blocks and add a few
missing section title identifiers.
One note with regards to "--": on Sphinx, this can't be used
to identify a list, as it will format it badly. This can be
used, however, to identify a long hyphen - and "---" is an
even longer one.
At its new index.rst, let's add a :orphan: while this is not linked to
the main index.rst file, in order to avoid build warnings.

The kmalloc() function has a 2-factor argument form, kmalloc_array(). This
patch replaces cases of:
kmalloc(a * b, gfp)
with:
kmalloc_array(a * b, gfp)
as well as handling cases of:
kmalloc(a * b * c, gfp)
with:
kmalloc(array3_size(a, b, c), gfp)
as it's slightly less ugly than:
kmalloc_array(array_size(a, b), c, gfp)
This does, however, attempt to ignore constant size factors like:
kmalloc(4 * 1024, gfp)
though any constants defined via macros get caught up in the conversion.
Any factors with a sizeof() of "unsigned char", "char", and "u8" were
dropped, since they're redundant.
The tools/ directory was manually excluded, since it has its own
implementation of kmalloc().

Now that the SPDX tag is in all tty files, that identifies the license
in a specific and legally-defined manner. So the extra GPL text wording
can be removed as it is no longer needed at all.
This is done on a quest to remove the 700+ different ways that files in
the kernel describe the GPL license text. And there's unneeded stuff
like the address (sometimes incorrect) for the FSF which is never
needed.
No copyright headers or other non-license-description text was removed.

It's good to have SPDX identifiers in all files to make it easier to
audit the kernel tree for correct licenses.
Update the drivers/tty files files with the correct SPDX license
identifier based on the license text in the file itself. The SPDX
identifier is a legally binding shorthand, which can be used instead of
the full boiler plate text.
This work is based on a script and data from Thomas Gleixner, Philippe
Ombredanne, and Kate Stewart.

vio_device_id are not supposed to change at runtime. All functions
working with vio_device_id provided by <asm/vio.h> work with
const vio_device_id. So mark the non-const structs as const.

"Here is the big driver core update for 4.13-rc1.
The large majority of this is a lot of cleanup of old fields in the
driver core structures and their remaining usages in random drivers.
All of those fixes have been reviewed by the various subsystem
maintainers. There's also some small firmware updates in here, a new
kobject uevent api interface that makes userspace interaction easier,
and a few other minor things.
All of these have been in linux-next for a long while with no reported
issues"

tty_wait_until_sent_from_close() drops the tty lock while waiting
for the tty driver to finish sending previously accepted data (ie.,
data remaining in its write buffer and transmit fifo).
tty_wait_until_sent_from_close() was added by commit a57a7bf3fc7e
("TTY: define tty_wait_until_sent_from_close") to prevent the entire
tty subsystem from being unable to open new ttys while waiting for
one tty to close while output drained.
However, since commit 0911261d4cb6 ("tty: Don't take tty_mutex for tty
count changes"), holding a tty lock while closing does not prevent other
ttys from being opened/closed/hung up, but only prevents lifetime event
changes for the tty under lock.
Holding the tty lock while waiting for output to drain does prevent
parallel non-blocking opens (O_NONBLOCK) from advancing or returning
while the tty lock is held. However, all parallel opens _already_
block even if the tty lock is dropped while closing and the parallel
open advances. Blocking in open has been in mainline since at least 2.6.29
(see tty_port_block_til_ready(); note the test for O_NONBLOCK is _after_
the wait while ASYNC_CLOSING).
IOW, before this patch a non-blocking open will sleep anyway for the
_entire_ duration of a parallel hardware shutdown, and when it wakes, the
error return will cause a release of its tty, and it will restart with
a fresh attempt to open. Similarly with a blocking open that is already
waiting; when it's woken, the hardware shutdown has already completed
to ASYNC_INITIALIZED is not set, which forces a release and restart as
well.
So, holding the tty lock across the _entire_ close (which is what this
patch does), even while waiting for output to drain, is equivalent to
the current outcome wrt parallel opens.

when strlen pi->location_code is larger than HVCS_CLC_LENGTH + 1,
original implementation can not let hvcsd->p_location_code NUL terminated.

Now, we start converting tty buffer functions to actually use
tty_port. This will allow us to get rid of the need of tty in many
call sites. Only tty_port will needed and hence no more
tty_port_tty_get in those paths.
Now, the one where most of tty_port_tty_get gets removed:
tty_flip_buffer_push.
IOW we also closed all the races in drivers not using tty_port_tty_get
at all yet.
Also we move tty_flip_buffer_push declaration from include/linux/tty.h
to include/linux/tty_flip.h to all others while we are changing it
anyway.

This has two outcomes:
* we give the TTY layer a tty_port
* we do not find the info structure every time open is called on that
tty
>From now on, we only increase the reference count in ->install (and

No refcounting, just a switch. The locking in the driver prevents
races, so in fact the refcounting is not needed. But while we have a
tty in tty_port, don't duplicate that and remove the one from
hvcs_struct.

This makes vio_register_driver() get the module owner & name at compile
time like PCI drivers do, and adds a name pointer directly in struct
vio_driver to avoid having to explicitly initialize the embedded
struct device.

Checking if tty->index is in bounds is not needed. The tty has the
index set in the initial open. This is done in get_tty_driver. And it
can be only in interval <0,driver->num).
So remove the tests which check exactly this interval. Some are
left untouched as they check against the current backing device count.
(Leaving apart that the check is racy in most of the cases.)

All num, magic and owner are set by alloc_tty_driver. No need to
re-set them on each allocation site.
pti driver sets something different to what it passes to
alloc_tty_driver. It is not a bug, since we don't use the lines
parameter in any way. Anyway this is fixed, and now we do the right
thing.

We transform the offenders into a test of irq <= 0 which will be ok while
the ARM people get their platform sorted. Once that is done (or in a while
if they don't do it anyway) then we will change them all to !irq checks.
For arch specific drivers that are already using NO_IRQ = 0 we just test
against zero so we don't need to re-review them later.

Since commit [e58aa3d2: genirq: Run irq handlers with interrupts disabled],
We run all interrupt handlers with interrupts disabled
and we even check and yell when an interrupt handler
returns with interrupts enabled (see commit [b738a50a:
genirq: Warn when handler enables interrupts]).
So now this flag is a NOOP and can be removed.

The Power platform requires the partner info buffer to be page aligned
otherwise it will fail the partner info hcall with H_PARAMETER. Switch
from using kmalloc to allocate this buffer to __get_free_page to ensure
page alignment.

Let's use the newly added helper to avoid stalls in drivers which are
not yet ported to tty_port helpers.
Those which are broken (call tty_wait_until_sent with irqs disabled)
are left untouched. They are in a deeper trouble than we are trying to
solve here. This includes amiserial, 68328serial, 68360serial and
crisv10.

The HVCS driver, for those who don't know, is a driver for the "server" side
of the IBM virtual terminal mechanism allowing Linux partitions to act as
terminal servers under IBM PowerVM hypervisor. It's almost never used on
the field at the moment.
However, it's part of our configs, and in its current incarnation, will
allocate the tty driver & major (with 64 minors) and create a kernel thread
whether it's used or not, ie, whether the hypervisor did put a virtual
terminal server device node in the partition or not (or whether running on
a pseries machine or not even).
This in turns causes modern distro's udev's to start trying to open all
those 64 minors at boot, which, since they aren't linked to anything,
causes the driver to spew errors in the kernel log for each of them.
Not nice.
This moves all that initialization to a function which is now only called
the first time a terminal server virtual IO device is actually probed
(that is almost never).
There's still a _LOT_ of cleanup that can be done in this driver, some
simple (almost all printk's statements in there shall either just be
removed or in some case turned into better written & more informative
messages, including using the dev_* variants etc...). This is left as
an exercise for whoever actually cares about that driver.
One could also try to be smart and dispose of all the tty related
resources when the last instance of the VIO server device
is removed (Hotplug anybody ?).

As requested by Arnd Bergmann, the hvc drivers are now
moved to the drivers/tty/hvc/ directory. The virtio_console.c driver
was also moved, as it required the hvc_console.h file to be able to be
built, and it really is a hvc driver.
