Pull SCSI fixes from James Bottomley:
"This is mostly fixes for driver specific issues (nine of them) and the
storvsc performance improvement with interrupt handling which was
dropped from the previous fixes pull request.
We also have two regressions: one is a double call_rcu() in ATA error
handling and the other is a missed conversion to BLK_STS_OK in
__scsi_error_from_host_byte()"

The data in NVRAM is not guaranteed to be NUL terminated. Since
snprintf expects byte-stream to accommodate null byte, the CHAP secret
is truncated. Use sprintf instead of snprintf to fix the truncation of
CHAP name and secret.

Advance the qed* drivers to use firmware 8.33.1.0:
Modify core driver (qed) to utilize the new FW and initialize the device
with it. This is the lion's share of the patch, and includes changes to FW
interface files, device initialization flows, FW interaction flows, and
debug collection flows.
Modify Ethernet driver (qede) to make use of new FW in fastpath.
Modify RoCE/iWARP driver (qedr) to make use of new FW in fastpath.
Modify FCoE driver (qedf) to make use of new FW in fastpath.

This patch refactors and reorders the FW API files in preparation of
upgrading the code to support new FW.
- Make use of the BIT macro in appropriate places.
- Whitespace changes to align values and code blocks.
- Comments are updated (spelling mistakes, removed if not clear).
- Group together code blocks which are related or deal with similar
matters.

The driver may sleep under a spinlock.
The function call path is:
qedi_cpu_offline (acquire the spinlock)
qedi_fp_process_cqes
qedi_mtask_completion
qedi_process_tmf_resp
kzalloc(GFP_KERNEL) --> may sleep
To fix it, GFP_KERNEL is replaced with GFP_ATOMIC.
This bug is found by my static analysis tool(DSAC) and checked by my
code review.

Out of order flow is not working for iWARP.
This patch got cut out from initial series that added out
of order support for iWARP.
Make out of order code common for iWARP and iSCSI.
Add new configuration option CONFIG_QED_OOO. Set by
qedr and qedi Kconfigs.

[qed_sp_iscsi_func_start:189(host_7-0)]Cannot satisfy CQ amount. Queues
requested 8, CQs available 4. Aborting function start
Above condition will resolve as management firmware is capable of
telling us the number of CQs available for a given PF, qed will
communicate the same number to qedi, So that qedi will know how much CQs
are allowed.

We shouldn't be writing over the "ret" variable. It means we return
ERR_PTR(0) which is NULL and it results in a NULL dereference in the
caller.

qedi uses iscsi_boot_sysfs to export the targets used for boot to
sysfs. Select the config option to make sure the module is built.

This patch adds support for Boot from SAN over iSCSI offload. The iSCSI
boot information in the NVRAM is populated under
/sys/firmware/iscsi_bootX/ using qed NVM-image reading API and further
exported to open-iscsi to perform iSCSI login enabling boot over offload
iSCSI interface in a Boot from SAN environment.

Current API between qed and protocol modules allows passing an
additional private string - but it doesn't get utilized by qed
anywhere.
Clarify the API by removing it and renaming it 'set_name'.

max_fin_rt is the maximum re-transmission of FIN packets
as part of the termination flow. After reaching this value
the FW will send a single RESET.

munmap done by iscsiuio during a stop of the service triggers a "bad
pte" warning sometimes. munmap kernel path goes through the mmapped
pages and has a validation check for mapcount (in struct page) to be
zero or above. kzalloc, which we had used to allocate udev->ctrl, uses
slab allocations, which re-uses mapcount (union) for other purposes that
can make the mapcount look negative. Avoid all these trouble by invoking
one of the __get_free_pages wrappers to be used instead of kzalloc for
udev->ctrl.

This pushes qed [and as result, all qed* drivers] into using 8.20.0.0
firmware. The changes are mostly contained in qed with minor changes
to qedi due to some HSI changes.
Content-wise, the firmware contains fixes to various issues exposed
since the release of the previous firmware, including:
- Corrects iSCSI fast retransmit when data digest is enabled.
- Stop draining packets when receiving several consecutive PFCs.
- Prevent possible assertion when consecutively opening/closing
many connections.
- Prevent possible assertion due to too long BDQ fetch time.
In addition, the new firmware would allow us to later add iWARP support
in qed and qedr.

Prepare to mark sensitive kernel structures for randomization by making
sure they're using designated initializers. These were identified during
allyesconfig builds of x86, arm, and arm64, with most initializer fixes
extracted from grsecurity.
For these cases, terminate the list with { }, which will be zero-filled,
instead of undesignated NULLs.

refcount_t type and corresponding API should be used instead of atomic_t
when the variable is used as a reference counter. This allows to avoid
accidental refcounter overflows that might lead to use-after-free
situations.

This patch advances the qed* drivers into using the newer firmware -
This solves several firmware bugs, mostly related [but not limited to]
various init/deinit issues in various offloaded protocols.
It also introduces a major 4-Cached SGE change in firmware, which can be
seen in the storage drivers' changes.
In addition, this firmware is required for supporting the new QL41xxx
series of adapters; While this patch doesn't add the actual support,
the firmware contains the necessary initialization & firmware logic to
operate such adapters [actual support would be added later on].

This defines the symbol as a constant in this case, as there is no way to
set it to anything other than zero without DEBUG_FS. In addition, I'm renaming
it to qedi_do_not_recover in order to put it into a driver specific namespace,
as "do_not_recover" is a really bad name for a kernel-wide global identifier
when it is used only in one driver.

The call to qedi_setup_int is not updating the return code rc yet rc is
being checked for an error. Fix this by assigning rc to the return code
from the call to qedi_setup_int.

'conn_info' is malloced in qedi_iscsi_update_conn() and should be freed
before leaving from the error handling cases, otherwise it will cause
memory leak.
