A bit of a brown paper bag issue. The previous patch to remove the soon
to be ripped out fields that were used in autosense actually broke the
driver. This patch fixes it and has been tested (honestly).

This one's quite tricky. The 53c700 uses the now deprecated fields of
the command structure for saving the prior command when it does
autosense. To fix this, we have to add extra fields to the LUN to
accommodate a copy of the real command.

max_id now means the maximum number of ids on the bus, which means it
is one greater than the largest possible id number.

Introduce new helpers:
- spi_populate_width_msg()
- spi_populate_sync_msg()
- spi_populate_ppr_msg()
and use them in drivers which already enable the SPI transport.

Wrap a highly common idiom. Makes the code easier to read, helps pave
the way for sdev->{id,channel} removal, and adds a token that can easily
by grepped-for in the future.
There are a couple sdev_id() and scmd_printk() updates thrown in as well.

Drivers need not implement a hook that returns FAILED, and does nothing
else, since the SCSI midlayer code will do that for us.

Initial git repository build. I'm not bothering with the full history,
even though we have it. We can create a separate "historical" git
archive of that later if we want to, and in the meantime it's about
3.2GB when imported into git - space that would just make the early
git days unnecessarily complicated, when we don't have a lot of good
infrastructure for it.
Let it rip!
