- After FW reset, IP addresses need to be reprogrammed to FW for LRO to work. This
was not happening in context reset path. Fixed it here.

Change the datatype of "ip_addr" to __be32 as 'ip' should be in
big endian format.
Adapter needs "ip address" in big endian format stored at lower 32bit
of req.word[1]. netxen_config_ipaddr() now receives 'ip' in big endian
format. To satisfy adapter's need, use memcpy() to copy byte by byte
of 'ip' into lower 32bit of req.word[1].
Mac address and serial number of adapter need to be in little endian format.
Change the data type of the related variables to __le32 / __le64 or cast it
explicitly to __le32 / __le64 depending upon the requirement.

In 'ql_adapter_initialize'
the first call for 'spin_unlock_irqrestore()' is with hw_flags = 0,
which is as good as 'spin_unlock_irq()' (unconditional interrupt
enabling). If this is intended, then for better performance
'spin_unlock_irqrestore()' can be replaced with 'spin_unlock_irq()'
and 'spin_lock_irqsave()' can be replaced by 'spin_lock_irq()

Unify return value of .ndo_set_mac_address if the given address
isn't valid. Return -EADDRNOTAVAIL as eth_mac_addr() already does
if is_valid_ether_addr() fails.

"esize" should be signed because it can be negative here. For example,
when we call it in netxen_parse_md_template(), it could be -1 from the
return value of netxen_md_L2Cache().

Let caller know the result of adding/removing vlan id to/from vlan
filter.
In some drivers I make those functions to just return 0. But in those
where there is able to see if hw setup went correctly, return value is
set appropriately.

When transmiting a fragmented skb, qlge fills a descriptor with the
fragment addresses, after DMA-mapping them. If there are more than eight
fragments, it will use the eighth descriptor as a pointer to an external
list. After mapping this external list, called OAL to a structure
containing more descriptors, it fills it with the extra fragments.
However, considering that systems with pages larger than 8KiB would have
less than 8 fragments, which was true before commit a715dea3c8e, it
defined a macro for the OAL size as 0 in those cases.
Now, if a skb with more than 8 fragments (counting skb->data as one
fragment), this would start overwriting the list of addresses already
mapped and would make the driver fail to properly unmap the right
addresses on architectures with pages larger than 8KiB.
Besides that, the list of mappings was one size too small, since it must
have a mapping for the maxinum number of skb fragments plus one for
skb->data and another for the OAL. So, even on architectures with page
sizes 4KiB and 8KiB, a skb with the maximum number of fragments would
make the driver overwrite its counter for the number of mappings, which,
again, would make it fail to unmap the mapped DMA addresses.

Instead, just checking for RESETTING_BIT not set before sending config_led
command down to card.
o Take rtnl_lock instead of RESETTING_BIT for beacon test while sending
config_led command down to make sure interface cannot be brought up/ down.
o Allocate and free resources if interface is down before
sending the config_led command. This is to make sure config_led
command sending doesn't fail.
o Clear QLCNIC_LED_ENABLE bit if beacon/ LED test fails to start.

In fw reset path, we should consider any change in device state as an
ack from the other driver. When that happens, we don't have to wait for
an explicit ack.

To ease skb->truesize sanitization, its better to be able to localize
all references to skb frags size.
Define accessors : skb_frag_size() to fetch frag size, and
skb_frag_size_{set|add|sub}() to manipulate it.
