This patch updates the contact information on the ixgbe driver files so
that every file includes the Linux NICS address, as it is still used,
but only a few of the files mentioned it.

Simplify the code. Attach hwmon sysfs attributes to hwmon device
instead of pci device. Avoid race conditions caused by attributes
being created after hwmon device registration. Implicitly
(through hwmon API) add mandatory 'name' sysfs attribute.
Other cleanup:
Instead of allocating memory for hwmon attributes, move attributes
and all other hwmon related data into struct hwmon_buff and allocate
the entire structure using devm_kzalloc.
Check return value from calls to igb_add_hwmon_attr() one by one instead
of logically combining them all together.

When the hwmon code was initially added it was with the assumption that a
sysfs patch would be also coming soon. Since that isn't the case some
clean up needs to be done. This patch does that.

While testing the TCP changes I had to fix an issue in order to be able to
load and unload the module.
The recent patch that added thermal sensor support added a use after free
bug on module unload with an 82598 adapter in the system. To resolve the
issue I have updated the code so that when we free the info_kobj we set it
back to NULL.
I suspect there are likely other bugs present, but I will leave that for
another patch that can undergo more testing.
I am submitting this directly to net-next since this fixes a fairly serious
bug that will lock up the ixgbe module until the system is rebooted.
