This fixes a race of the form:
CPU0 CPU1
mlx5_ib_invalidate_range() mlx5_ib_invalidate_range()
// This one actually makes npages == 0
ib_umem_odp_unmap_dma_pages()
if (npages == 0 && !dying)
// This one does nothing
ib_umem_odp_unmap_dma_pages()
if (npages == 0 && !dying)
dying = 1;
dying = 1;
schedule_work(&umem_odp->work);
// Double schedule of the same work
schedule_work(&umem_odp->work); // BOOM
npages and dying must be read and written under the umem_mutex lock.
Since whenever ib_umem_odp_unmap_dma_pages() is called mlx5 must also call
mlx5_ib_update_xlt, and both need to be done in the same locking region,
hoist the lock out of unmap.
This avoids an expensive double critical section in
mlx5_ib_invalidate_range().

rdma_for_each_port is already incrementing the iterator's value it
receives therefore, after the first iteration the iterator is increased by
2 which eventually causing wrong queries and possible traces.
Fix the above by removing the old redundant incrementation that was used

In the process of moving the debug counters sysfs entries, the commit
mentioned below eliminated the cm_infiniband sysfs directory.
This sysfs directory was tied to the cm_port object allocated in procedure
cm_add_one().
Before the commit below, this cm_port object was freed via a call to
kobject_put(port->kobj) in procedure cm_remove_port_fs().
Since port no longer uses its kobj, kobject_put(port->kobj) was eliminated.
This, however, meant that kfree was never called for the cm_port buffers.
Fix this by adding explicit kfree(port) calls to functions cm_add_one()
and cm_remove_one().
Note: the kfree call in the first chunk below (in the cm_add_one error
flow) fixes an old, undetected memory leak.

[11~From: John Hubbard <jhubbard@nvidia.com>
Subject: mm/gup: add make_dirty arg to put_user_pages_dirty_lock()
Patch series "mm/gup: add make_dirty arg to put_user_pages_dirty_lock()",
v3.
There are about 50+ patches in my tree [2], and I'll be sending out the
remaining ones in a few more groups:
* The block/bio related changes (Jerome mostly wrote those, but I've had
to move stuff around extensively, and add a little code)
* mm/ changes
* other subsystem patches
* an RFC that shows the current state of the tracking patch set. That
can only be applied after all call sites are converted, but it's good to
get an early look at it.
This is part a tree-wide conversion, as described in fc1d8e7cca2d ("mm:
introduce put_user_page*(), placeholder versions").
This patch (of 3):
Provide more capable variation of put_user_pages_dirty_lock(), and delete
put_user_pages_dirty(). This is based on the following:
1. Lots of call sites become simpler if a bool is passed into
put_user_page*(), instead of making the call site choose which
put_user_page*() variant to call.
2. Christoph Hellwig's observation that set_page_dirty_lock() is
usually correct, and set_page_dirty() is usually a bug, or at least
questionable, within a put_user_page*() calling chain.
This leads to the following API choices:
* put_user_pages_dirty_lock(page, npages, make_dirty)
* There is no put_user_pages_dirty(). You have to
hand code that, in the rare case that it's
required.

https://lore.kernel.org/netdev/alpine.LFD.2.21.1909151104060.2546@ja.home.ssi.bg/
Revert that part of the commit referenced in the Fixes tag.
Currently, there are no u8 holes in 'struct rtable'. There is a 4-byte hole
in the second cacheline which contains the gateway declaration. So move
rt_gw_family down to the gateway declarations since they are always used
together, and then re-use that u8 for rt_uses_gateway. End result is that
rtable size is unchanged.

The cited commit introduced a double-free of the srq buffer in the error

The problem is that ib_destroy_srq_user() called in the error flow also
frees the srq buffer.
Thus, if uverbs_response() fails in __uverbs_create_srq(), the srq buffer

At this point the ucontext is only being stored to access the ib_device,
so just store the ib_device directly instead. This is more natural and
logical as the umem has nothing to do with the ucontext.

the existing overflow checks done by ib_umem_get() are not

Further, remove the unchecked math from the inlines and just use the

The three paths to build the umem_odps are kind of muddled, they are:
- As a normal ib_mr umem
- As a child in an implicit ODP umem tree
- As the root of an implicit ODP umem tree
Only the first two are actually umem's, the last is an abuse.
The implicit case can only be triggered by explicit driver request, it
should never be co-mingled with the normal case. While we are here, make
sensible function names and add some comments to make this clearer.

ODP is working with userspace VA's in the interval tree which always fit
into an unsigned long, so we can use the common code.
This comes at a cost of a 16 byte increase in ib_umem_odp struct size due
to storing the interval tree start/last in addition to the umem
addr/length. However these values were computed and are performance
critical for the interval lookup, so this seems like a worthwhile trade
off.

Once implicit MR is being called to be released by
ib_umem_notifier_release() its leaves were marked as "dying".
However, when dereg_mr()->mlx5_ib_free_implicit_mr()->mr_leaf_free() is
called, it skips running the mr_leaf_free_action (i.e. umem_odp->work)
when those leaves were marked as "dying".
As such ib_umem_release() for the leaves won't be called and their MRs
will be leaked as well.
When an application exits/killed without calling dereg_mr we might hit the
above flow.
This fatal scenario is reported by WARN_ON() upon
mlx5_ib_dealloc_ucontext() as ibcontext->per_mm_list is not empty, the
call trace can be seen below.
Originally the "dying" mark as part of ib_umem_notifier_release() was
introduced to prevent pagefault_mr() from returning a success response
once this happened. However, we already have today the completion
mechanism so no need for that in those flows any more. Even in case a
success response will be returned the firmware will not find the pages and
an error will be returned in the following call as a released mm will
cause ib_umem_odp_map_dma_pages() to permanently fail mmget_not_zero().
Fix the above issue by dropping the "dying" from the above flows. The
other flows that are using "dying" are still needed it for their
synchronization purposes.

Send and Receive completion is handled on a single CPU selected at
the time each Completion Queue is allocated. Typically this is when
an initiator instantiates an RDMA transport, or when a target
accepts an RDMA connection.
Some ULPs cannot open a connection per CPU to spread completion
workload across available CPUs and MSI vectors. For such ULPs,
provide an API that allows the RDMA core to select a completion
vector based on the device's complement of available comp_vecs.
ULPs that invoke ib_alloc_cq() with only comp_vector 0 are converted
to use the new API so that their completion workloads interfere less
with each other.

The problem was that the MAD PD was deallocated before the MAD CQ.
There was completion work pending for the CQ when the PD got deallocated.
When the mad completion handling reached procedure
ib_mad_post_receive_mads(), we got a use-after-free bug in the following
line of code in that procedure:
sg_list.lkey = qp_info->port_priv->pd->local_dma_lkey;
(the pd pointer in the above line is no longer valid, because the
pd has been deallocated).
We fix this by allocating the PD before the CQ in procedure
ib_mad_port_open(), and deallocating the PD after freeing the CQ
in procedure ib_mad_port_close().
Since the CQ completion work queue is flushed during ib_free_cq(),
no completions will be pending for that CQ when the PD is later
deallocated.
Note that freeing the CQ before deallocating the PD is the practice
in the ULPs.

The check for QP type different than XRC has excluded driver QP
types from the resource tracker.
As a result, "rdma resource show" user command would not show opened
driver QPs which does not reflect the real state of the system.
Check QP type explicitly instead of assuming enum values/ordering.

Due to the complexity of client->remove() callbacks it is desirable to not
hold any locks while calling them. Remove the last one by tracking only
the highest client ID and running backwards from there over the xarray.
Since the only purpose of that lock was to protect the linked list, we can
drop the lock.

Which is due to the read side of the client_data_rwsem being obtained
recursively through a work queue flush during cm client removal.
The lock is being held across the remove in remove_client_context() so
that the function is a fence, once it returns the client is removed. This
is required so that the two callers do not proceed with destruction until
the client completes removal.
Instead of using client_data_rwsem use the existing device unregistration
refcount and add a similar client unregistration (client->uses) refcount.
This will fence the two unregistration paths without holding any locks.

Some processors may mispredict an array bounds check and
speculatively access memory that they should not. With
a user supplied array index we like to play things safe
by masking the value with the array size before it is
used as an index.
