This patch adds missing MODULE_DEVICE_TABLE definition which generates
correct modalias for automatic loading of this driver when it is built
as an external module.

Move led_mode attribute from HID device to led-class devices and
rename it mode. This will also fix race condition by using
attribute-groups.
