2012-11-23s390/ptrace: race of single stepping vs signal deliveryMartin Schwidefsky
The current single step code is racy in regard to concurrent delivery of signals. If a signal is delivered after a PER program check occurred but before the TIF_PER_TRAP bit has been checked in entry[64].S the code clears TIF_PER_TRAP and then calls do_signal. This is wrong, if the instruction completed (or has been suppressed) a SIGTRAP should be delivered to the debugger in any case. Only if the instruction has been nullified the SIGTRAP may not be send. The new logic always sets TIF_PER_TRAP if the program check indicates PER tracing but removes it again for all program checks that are nullifying. The effect is that for each change in the PSW address we now get a single SIGTRAP. Reported-by: Andreas Arnez <arnez@linux.vnet.ibm.com> Signed-off-by: Martin Schwidefsky <schwidefsky@de.ibm.com>

2012-11-23s390/mm: keep fault_init() private to fault.cHeiko Carstens
Just convert fault_init() to an early initcall. That's still early enough since it only needs be called before user space processes get executed. No reason to externalize it. Also add the function to the init section and move the store_indication variable to the read_mostly section. 2012-11-23s390/mm,vmemmap: use 1MB frames for vmemmapHeiko Carstens
Use 1MB frames for vmemmap if EDAT1 is available in order to reduce TLB pressure Always use a 1MB frame even if its only partially needed for struct pages. Otherwise we would end up with a mix of large frame and page mappings, because vmemmap_populate gets called for each section (256MB -> 3.5MB memmap) separately. Worst case is that we would waste 512KB. Signed-off-by: Heiko Carstens <heiko.carstens@de.ibm.com> Signed-off-by: Martin Schwidefsky <schwidefsky@de.ibm.com>

2012-11-13s390/gup: fix access_ok() usage in __get_user_pages_fast()Heiko Carstens
access_ok() returns always "true" on s390. Therefore all access_ok() invocations are rather pointless. However when walking page tables we need to make sure that everything is within bounds of the ASCE limit of the task's address space. So remove the access_ok() call and add the same check we have in get_user_pages_fast(). 2012-11-13s390/gup: add missing TASK_SIZE check to get_user_pages_fast()Heiko Carstens
When walking page tables we need to make sure that everything is within bounds of the ASCE limit of the task's address space. Otherwise we might calculate e.g. a pud pointer which is not within a pud and dereference it. So check against TASK_SIZE (which is the ASCE limit) before walking page tables. 2012-10-26s390/mm: use pmd_large() instead of pmd_huge()Gerald Schaefer
Without CONFIG_HUGETLB_PAGE, pmd_huge() will always return 0. So pmd_large() should be used instead in places where both transparent huge pages and hugetlbfs pages can occur. Signed-off-by: Gerald Schaefer <gerald.schaefer@de.ibm.com> Signed-off-by: Martin Schwidefsky <schwidefsky@de.ibm.com>
2012-10-09s390/mm,vmem: fix vmem_add_mem()/vmem_remove_range()Heiko Carstens
vmem_add_mem() should only then insert a large page if pmd_none() is true for the specific entry. We might have a leftover from a previous mapping. 2012-10-09s390/vmalloc: have separate modules areaHeiko Carstens
Add a special module area on top of the vmalloc area, which may be only used for modules and bpf jit generated code. 2012-10-09s390/mm: fix mapping of read-only kernel text sectionHeiko Carstens
Within the identity mapping the kernel text section is mapped read-only. However when mapping the first and last page of the text section we must round upwards and downwards respectively, if only parts of a page belong to the section. Otherwise potential rw data can be mapped read-only. So the rounding must be done just the other way we have it right now. 2012-10-09s390/mm: add page table dumperHeiko Carstens
This is more or less the same as the x86 page table dumper which was merged four years ago: 926e5392 "x86: add code to dump the (kernel) page tables for visual inspection by kernel developers". We add a file at /sys/kernel/debug/kernel_page_tables for debugging purposes so it's quite easy to see the kernel page table layout and possible odd mappings: ---[ Identity Mapping ]--- 0x0000000000000000-0x0000000000100000 1M PTE RW ---[ Kernel Image Start ]--- 0x0000000000100000-0x0000000000800000 7M PMD RO 0x0000000000800000-0x00000000008a9000 676K PTE RO 0x00000000008a9000-0x0000000000900000 348K PTE RW 0x0000000000900000-0x0000000001500000 12M PMD RW ---[ Kernel Image End ]--- 0x0000000001500000-0x0000000280000000 10219M PMD RW 0x0000000280000000-0x000003d280000000 3904G PUD I ---[ vmemmap Area ]--- 0x000003d280000000-0x000003d288c00000 140M PTE RW 0x000003d288c00000-0x000003d300000000 1908M PMD I 0x000003d300000000-0x000003e000000000 52G PUD I ---[ vmalloc Area ]--- 0x000003e000000000-0x000003e000009000 36K PTE RW 0x000003e000009000-0x000003e0000ee000 916K PTE I 0x000003e0000ee000-0x000003e000146000 352K PTE RW 0x000003e000146000-0x000003e000200000 744K PTE I 0x000003e000200000-0x000003e080000000 2046M PMD I 0x000003e080000000-0x0000040000000000 126G PUD I This usually makes only sense for kernel developers. 2012-10-09s390/mm: fix pmd_huge() usage for kernel mappingHeiko Carstens
pmd_huge() will always return 0 on !HUGETLBFS, however we use that helper function when walking the kernel page tables to decide if we have a 1MB page frame or not. Since we create 1MB frames for the kernel 1:1 mapping independently of HUGETLBFS this can lead to incorrect storage accesses since the code can assume that we have a pointer to a page table instead of a pointer to a 1MB frame. Fix this by adding a pmd_large() primitive like other architectures have it already and remove all references to HUGETLBFS/HUGETLBPAGE from the code that walks kernel page tables. 2012-10-09readahead: fault retry breaks mmap file read random detectionShaohua Li
.fault now can retry. The retry can break state machine of .fault. In filemap_fault, if page is miss, ra->mmap_miss is increased. In the second try, since the page is in page cache now, ra->mmap_miss is decreased. And these are done in one fault, so we can't detect random mmap file access. Add a new flag to indicate .fault is tried once. In the second try, skip ra->mmap_miss decreasing. The filemap_fault state machine is ok with it. I only tested x86, didn't test other archs, but looks the change for other archs is obvious, but who knows :) Signed-off-by: Shaohua Li <shaohua.li@fusionio.com> Cc: Rik van Riel <riel@redhat.com> Cc: Wu Fengguang <fengguang.wu@intel.com> Signed-off-by: Andrew Morton <akpm@linux-foundation.org> Signed-off-by: Linus Torvalds <torvalds@linux-foundation.org>

This patch is part of the architecture backend for thp on s390. It disables thp for kvm hosts, because there is no kvm host hugepage support so far. 2012-10-09thp, s390: thp pagetable pre-allocation for s390Gerald Schaefer
This patch is part of the architecture backend for thp on s390. It provides the pagetable pre-allocation functions pgtable_trans_huge_deposit() and pgtable_trans_huge_withdraw(). Unlike other archs, s390 has no struct page * as pgtable_t, but rather a pointer to the page table. So instead of saving the pagetable pre- allocation list info inside the struct page, it is being saved within the pagetable itself. 2012-07-30s390/mm: fix fault handling for page table walk caseHeiko Carstens
Make sure the kernel does not incorrectly create a SIGBUS signal during user space accesses: For user space accesses in the switched addressing mode case the kernel may walk page tables and access user address space via the kernel mapping. If a page table entry is invalid the function __handle_fault() gets called in order to emulate a page fault and trigger all the usual actions like paging in a missing page etc. by calling handle_mm_fault(). If handle_mm_fault() returns with an error fixup handling is necessary. For the switched addressing mode case all errors need to be mapped to -EFAULT, so that the calling uaccess function can return -EFAULT to user space. Unfortunately the __handle_fault() incorrectly calls do_sigbus() if VM_FAULT_SIGBUS is set. This however should only happen if a page fault was triggered by a user space instruction. For kernel mode uaccesses the correct action is to only return -EFAULT. So user space may incorrectly see SIGBUS signals because of this bug. For current machines this would only be possible for the switched addressing mode case in conjunction with futex operations. 2012-05-16s390/pfault: fix task state raceHeiko Carstens
When setting the current task state to TASK_UNINTERRUPTIBLE this can race with a different cpu. The other cpu could set the task state after it inspected it (while it was still TASK_RUNNING) to TASK_RUNNING which would change the state from TASK_UNINTERRUPTIBLE to TASK_RUNNING again. This race was always present in the pfault interrupt code but didn't cause anything harmful before commit f2db2e6c "[S390] pfault: cpu hotplug vs missing completion interrupts" which relied on the fact that after setting the task state to TASK_UNINTERRUPTIBLE the task would really sleep. Since this is not necessarily the case the result may be a list corruption of the pfault_list or, as observed, a use-after-free bug while trying to access the task_struct of a task which terminated itself already. To fix this, we need to get a reference of the affected task when receiving the initial pfault interrupt and add special handling if we receive yet another initial pfault interrupt when the task is already enqueued in the pfault list. 2012-05-16s390: allow absolute memory access for /dev/memMichael Holzheu
Currently dev/mem for s390 provides only real memory access. This means that the CPU prefix pages are swapped. The prefix swap for real memory works as follows: Each CPU owns a prefix register that points to a page aligned memory location "P". If this CPU accesses the address range [0,0x1fff], it is translated by the hardware to [P,P+0x1fff]. Accordingly if this CPU accesses the address range [P,P+0x1fff], it is translated by the hardware to [0,0x1fff]. Therefore, if [P,P+0x1fff] or [0,0x1fff] is read from the current /dev/mem device, the incorrectly swapped memory content is returned. With this patch the /dev/mem architecture code is modified to provide absolute memory access. This is done via the arch specific functions xlate_dev_mem_ptr() and unxlate_dev_mem_ptr(). For swapped pages on s390 the function xlate_dev_mem_ptr() now returns a new buffer with a copy of the requested absolute memory. In case the buffer was allocated, the unxlate_dev_mem_ptr() function frees it after /dev/mem code has called copy_to_user(). 2012-04-11[S390] fix tlb flushing for page table pagesMartin Schwidefsky
Git commit 36409f6353fc2d7b6516e631415f938eadd92ffa "use generic RCU page-table freeing code" introduced a tlb flushing bug. Partially revert the above git commit and go back to s390 specific page table flush code. For s390 the TLB can contain three types of entries, "normal" TLB page-table entries, TLB combined region-and-segment-table (CRST) entries and real-space entries. Linux does not use real-space entries which leaves normal TLB entries and CRST entries. The CRST entries are intermediate steps in the page-table translation called translation paths. For example a 4K page access in a three-level page table setup will create two CRST TLB entries and one page-table TLB entry. The advantage of that approach is that a page access next to the previous one can reuse the CRST entries and needs just a single read from memory to create the page-table TLB entry. The disadvantage is that the TLB flushing rules are more complicated, before any page-table may be freed the TLB needs to be flushed. In short: the generic RCU page-table freeing code is incorrect for the CRST entries, in particular the check for mm_users < 2 is troublesome. This is applicable to 3.0+ kernels. 2012-04-11[S390] kernel: Use local_irq_save() for memcpy_real()Michael Holzheu
Currently in the memcpy_real() function interrupts are disabled with __arch_local_irq_stnsm(). In order to notify lockdep that interrupts are disabled, with this patch local_irq_save() is used instead. The function __arch_local_irq_stnsm() is still used for switching to real mode. git://git.kernel.org/pub/scm/linux/kernel/git/dhowells/linux-asm_system Pull "Disintegrate and delete asm/system.h" from David Howells: "Here are a bunch of patches to disintegrate asm/system.h into a set of separate bits to relieve the problem of circular inclusion dependencies. I've built all the working defconfigs from all the arches that I can and made sure that they don't break. The reason for these patches is that I recently encountered a circular dependency problem that came about when I produced some patches to optimise get_order() by rewriting it to use ilog2(). This uses bitops - and on the SH arch asm/bitops.h drags in asm-generic/get_order.h by a circuituous route involving asm/system.h. The main difficulty seems to be asm/system.h. It holds a number of low level bits with no/few dependencies that are commonly used (eg. memory barriers) and a number of bits with more dependencies that aren't used in many places (eg. switch_to()). These patches break asm/system.h up into the following core pieces: (1) asm/barrier.h Move memory barriers here. This already done for MIPS and Alpha. (2) asm/switch_to.h Move switch_to() and related stuff here. (3) asm/exec.h Move arch_align_stack() here. Other process execution related bits could perhaps go here from asm/processor.h. (4) asm/cmpxchg.h Move xchg() and cmpxchg() here as they're full word atomic ops and frequently used by atomic_xchg() and atomic_cmpxchg(). (5) asm/bug.h Move die() and related bits. (6) asm/auxvec.h Move AT_VECTOR_SIZE_ARCH here. Other arch headers are created as needed on a per-arch basis." Fixed up some conflicts from other header file cleanups and moving code around that has happened in the meantime, so David's testing is somewhat weakened by that. 2012-03-11[S390] irq: external interrupt code passingHeiko Carstens
The external interrupt handlers have a parameter called ext_int_code. Besides the name this paramter does not only contain the ext_int_code but in addition also the "cpu address" (POP) which caused the external interrupt. To make the code a bit more obvious pass a struct instead so the called function can easily distinguish between external interrupt code and cpu address. The cpu address field however is named "subcode" since some external interrupt sources do not pass a cpu address but a different parameter (or none at all).
