The existing implementation lets machine_kexec() CPU jump to reboot code
buffer, whereas other CPUs to relocated_kexec_smp_wait. The natural way to
bring up an SMP new kernel would be to let CPU0 do it while others being
halted. For those failing to do so, fall back to the jumping method.

Commit a3e6c1eff548 ("MIPS: IRQ: Fix disable_irq on CPU IRQs") fixes an
issue where disable_irq did not actually disable the irq. The bug caused
our IPIs to not be disabled, which actually is the correct behavior.
With the addition of commit a3e6c1eff548 ("MIPS: IRQ: Fix disable_irq on
CPU IRQs"), the IPIs were getting disabled going into suspend, thus
schedule_ipi() was not being called. This caused deadlocks where
schedulable task were not being scheduled and other cpus were waiting
for them to do something.
Add the IRQF_NO_SUSPEND flag so an irq_disable will not be called on the
IPIs during suspend.

Remove the __init annotation from bmips_cpu_setup() to avoid the
following warning.
WARNING: vmlinux.o(.text+0x35c950): Section mismatch in reference from the function brcmstb_pm_s3() to the function .init.text:bmips_cpu_setup()
The function brcmstb_pm_s3() references
the function __init bmips_cpu_setup().
This is often because brcmstb_pm_s3 lacks a __init
annotation or the annotation of bmips_cpu_setup is wrong.

Allow the boot_secondary SMP op to return an error to __cpu_up(), which
will in turn return it to its caller.
This will allow SMP implementations to return errors quickly in cases
they they know have failed, rather than relying upon __cpu_up()
eventually timing out waiting for the cpu_running completion.

We currently have fields in struct cpuinfo_mips for the core & VP(E) ID
of a particular CPU, and various pieces of code directly access those
fields. This patch abstracts such access by introducing accessor
functions cpu_core(), cpu_set_core(), cpu_vpe_id() & cpu_set_vpe_id()
and having code that needs to access these values call those functions
rather than directly accessing the struct cpuinfo_mips fields. This
prepares us for changes to the way in which those values are stored in
later patches.
The cpu_vpe_id() function is introduced even though we already had a
cpu_vpe_id() macro for a couple of reasons:
1) It's more consistent with the core, and future cluster, accessors.
2) It ensures a sensible return type without explicit casts.
3) It's generally preferable to use functions rather than macros.

smp_ops providers do not modify their ops structures, so they should be
made const for robustness. Since currently the MIPS kernel is not mapped
with memory protection, this does not in itself provide any security
benefit, but it still makes sense to make this change.
There are also slight code size efficincies from the structure being
made read-only, saving 128 bytes of kernel text on a
pistachio_defconfig.

After the split of linux/sched.h, several platforms in arch/mips stopped building.
Add the respective additional #include statements to fix the problem I first
tried adding these into asm/processor.h, but ran into circular header
dependencies with that which I could not figure out.
The commit I listed as causing the problem is the branch merge, as there is
likely a combination of multiple patches in that branch.

We are going to split <linux/sched/hotplug.h> out of <linux/sched.h>, which
will have to be picked up from other headers and a couple of .c files.
Create a trivial placeholder <linux/sched/hotplug.h> file that just
maps to <linux/sched.h> to make this patch obviously correct and
bisectable.
Include the new header in the files that are going to need it.

While we properly disabled the per-CPU timer interrupt, we also need to
make sure that all interrupts that can possibly have this CPU in their
smp_affinity mask also have a chance to see this interrupt migrated to a
CPU not being taken offline.

When a CPU is disabled via CPU hotplug, cpu_foreign_map is not updated.
This could result in cache management SMP calls being sent to offline
CPUs instead of online siblings in the same core.
Add a call to calculate_cpu_foreign_map() in the various MIPS cpu
disable callbacks after set_cpu_online(). All cases are updated for
consistency and to keep cpu_foreign_map strictly up to date, not just
those which may support hardware multithreading.

Port the stblinux-3.3 code to perform a bunch of CPU-specific initialization,
make it compatible with run-time detection of the CPU, and unroll the
brcmstb-specific macros: BDEV_RB(), BDEV_UNSET.
The "pref 30" disabling is done as a quirk. This is a preliminary change to
allow the use of the "rotr" instruction gated by cpu_has_rixi.

The majority of SMP platforms handle their IPIs through do_IRQ()
which calls irq_{enter/exit}(). When a call function IPI is received,
smp_call_function_interrupt() is called which also calls
irq_{enter,exit}(), meaning irq_count is raised twice.
When tick broadcasting is used (which is implemented via a call
function IPI), this incorrectly causes all CPU idle time on the core
receiving broadcast ticks to be accounted as time spent servicing
IRQs, as account_process_tick() will account as such if irq_count is
greater than 1. This results in 100% CPU usage being reported on a
core which receives its ticks via broadcast.
This patch removes the SMP smp_call_function_interrupt() wrapper which
calls irq_{enter,exit}(). Platforms which handle their IPIs through
do_IRQ() now call generic_smp_call_function_interrupt() directly to
avoid incrementing irq_count a second time. Platforms which don't
(loongson, sgi-ip27, sibyte) call generic_smp_call_function_interrupt()
wrapped in irq_{enter,exit}().

On some chips like bcm3384, "other stuff" gets wired up to CPU1's IE_IRQ1
input, generating spurious IRQs. In this case we want the platform code
to be able to mask it off.

The secondary CPU's reset vector needs to be set to KSEG1 for a cold
boot (release from reset), or KSEG0 for a warm restart. On a cold boot
KSEG0 may be unavailable (BMIPS4380), and on a warm restart KSEG1 may
be unavailable (XKS01 mode on 4380 or 5000).

CPU interrupts need to be disabled on a cpu being taken down.
When a cpu is hot-plugged out of the system the following sequence occurs.
On the CPU where the hotplug sequence was initiated:

Before the cpu_notifier is called with CPU_DYING, all interrupts on the
dying cpu must be disabled. This guarantees that before tick_notify is
called with the CPU_DYING event and sets the clock device pointer to
NULL, there can not be any more clock interrupts.
When this wasn't done, an unfortunately-timed timer interrupt sometimes
caused hangs immediately prior to system suspend:

BMIPS3300 processors do not have the hardware to support SMP, but with a
small tweak, the SMP ebase relocation code allows BMIPS3300-based
platforms to reuse the S2/S3 power management code from BMIPS4380-based
chips. Normally this is as simple as adding one line to prom_init():

On some older BMIPS5200 (dual core / quad thread) platforms, the
PROM code set up CPU2/CPU3 so they would be started through an NMI
instead of through the ACTION register. But this was incompatible with
some power management features that were later added, so the scheme was
changed so that Linux is fully responsible for booting CPU2/CPU3.

__get_cpu_var() is used for multiple purposes in the kernel source. One of
them is address calculation via the form &__get_cpu_var(x). This calculates
the address for the instance of the percpu variable of the current processor
based on an offset.
Other use cases are for storing and retrieving data from the current
processors percpu area. __get_cpu_var() can be used as an lvalue when
writing data or on the right side of an assignment.
__get_cpu_var() is defined as :
#define __get_cpu_var(var) (*this_cpu_ptr(&(var)))
__get_cpu_var() always only does an address determination. However, store
and retrieve operations could use a segment prefix (or global register on
other platforms) to avoid the address calculation.
this_cpu_write() and this_cpu_read() can directly take an offset into a
percpu area and use optimized assembly code to read and write per cpu
variables.
This patch converts __get_cpu_var into either an explicit address
calculation using this_cpu_ptr() or into a use of this_cpu operations that
use the offset. Thereby address calculations are avoided and less registers
are used when code is generated.
At the end of the patch set all uses of __get_cpu_var have been removed so
the macro is removed too.
The patch set includes passes over all arches as well. Once these operations
are used throughout then specialized macros can be defined in non -x86
arches as well in order to optimize per cpu access by f.e. using a global
register that may be set to the per cpu base.
Transformations done to __get_cpu_var()
1. Determine the address of the percpu instance of the current processor.

int *x = this_cpu_ptr(&y);
2. Same as #1 but this time an array structure is involved.

3. Retrieve the content of the current processors instance of a per cpu
variable.
DEFINE_PER_CPU(int, y);
int x = __get_cpu_var(y)
Converts to
int x = __this_cpu_read(y);
4. Retrieve the content of a percpu struct
DEFINE_PER_CPU(struct mystruct, y);
struct mystruct x = __get_cpu_var(y);
Converts to
memcpy(&x, this_cpu_ptr(&y), sizeof(x));
5. Assignment to a per cpu variable
DEFINE_PER_CPU(int, y)
__get_cpu_var(y) = x;
Converts to
__this_cpu_write(y, x);
6. Increment/Decrement etc of a per cpu variable
DEFINE_PER_CPU(int, y);
__get_cpu_var(y)++

Allow building for all bmips cpus at the same time by changing ifdefs
to checks for the cpu type, or adding appropriate checks to the
assembly.
Since BMIPS43XX and BMIPS5000 require different IPI implementations,
split the SMP ops into one for each, so the runtime overhead is only
at registration time for them.

The hardware interrupt routing for boot CPU != 0 is wrong because it
will route all the hardware interrupts to TP0 which is not the one we
booted from. Fix this by properly checking which boot CPU we are booting
from and updating the right interrupt mask for the boot CPU. This fixes
booting on BCM3368 with bmips_smp_emabled = 0.

The current BMIPS SMP code assumes that the slave CPU is physical and
logical CPU 1, but on some systems such as BCM3368, the slave CPU is
physical CPU0. Fix the code to read the physical CPU (thread ID) we are
running this code on, and adjust the relocation vector address based on
it. This allows bringing up the second CPU on BCM3368 for instance.

Commit 4df715aa ("MIPS: BMIPS: support booting from physical CPU other
than 0") changed the interupt routing when we are booting from physical
CPU 0, but the settings are actually correct if we are booting from
physical CPU 0 or CPU 1. Revert that specific change.

Pull MIPS fixes from Ralf Baechle:
"MIPS fixes for 3.11. Half of then is for Netlogic the remainder
touches things across arch/mips.
Nothing really dramatic and by rc1 standards MIPS will be in fairly

Commit 4df715aa ["MIPS: BMIPS: support booting from physical CPU other
than 0"] introduced a thinko which will prevents slave CPUs from being
released from reset on systems where we boot from TP0. The problem is
that we are checking whether the slave CPU logical CPU map is 0, which
is never true for systems booting from TP0, so we do not release the
slave TP from reset and we are just stuck. Fix this by properly checking
that the CPU we intend to boot really is the physical slave CPU (logical
and physical value being 1).

The __cpuinit type of throwaway sections might have made sense
some time ago when RAM was more constrained, but now the savings
do not offset the cost and complications. For example, the fix in
commit 5e427ec2d0 ("x86: Fix bit corruption at CPU resume time")
is a good example of the nasty type of bugs that can be created
with improper use of the various __init prefixes.
After a discussion on LKML[1] it was decided that cpuinit should go
the way of devinit and be phased out. Once all the users are gone,
we can then finally remove the macros themselves from linux/init.h.
Note that some harmless section mismatch warnings may result, since
notify_cpu_starting() and cpu_up() are arch independent (kernel/cpu.c)
and are flagged as __cpuinit -- so if we remove the __cpuinit from
the arch specific callers, we will also get section mismatch warnings.
As an intermediate step, we intend to turn the linux/init.h cpuinit
related content into no-ops as early as possible, since that will get
rid of these warnings. In any case, they are temporary and harmless.
Here, we remove all the MIPS __cpuinit from C code and __CPUINIT
from asm files. MIPS is interesting in this respect, because there
are also uasm users hiding behind their own renamed versions of the
__cpuinit macros.

BMIPS43xx CPUs have two hardware threads, and on some SoCs such as 3368,
the bootloader has configured the system to boot from TP1 instead of the
more usual TP0. Create the physical to logical CPU mapping to cope with
that, do not remap the software interrupts to be cross CPUs such that we
do not have to do use the logical CPU mapping further down the code, and
finally, reset the slave TP1 only if booted from TP0.
