add_memory_region was the old interface for registering memory and
was already changed to used memblock internaly. Replace it by
directly calling memblock functions.

Instead of including all Platform files, we simply include the
needed one and avoid clashes with makefile variables.

request_irq() is preferred over setup_irq(). Invocations of setup_irq()
occur after memory allocators are ready.
Per tglx[1], setup_irq() existed in olden days when allocators were not
ready by the time early interrupts were initialized.
Hence replace setup_irq() by request_irq().
remove_irq() has been replaced by free_irq() as well.
There were build error's during previous version, couple of which was
reported by kbuild test robot <lkp@intel.com> of which one was reported
by Thomas Bogendoerfer <tsbogend@alpha.franken.de> as well. There were a
few more issues including build errors, those also have been fixed.

As of commit 34b1252bd91851f77f89fbb6829a04efad900f41 ("MIPS:
Cobalt: Do not build MTD platform device registration code as module.")
this file became built-in instead of modular. So we should also
stop using module_init as an alias for __initcall as that can be
rather misleading.
Fix this up now, so that we can relocate module_init from
init.h into module.h in the future. If we don't do this, we'd
have to add module.h to obviously non-modular code, and that
would be a worse thing.
Direct use of __initcall is discouraged, vs prioritized ones.
Use of device_initcall is consistent with what __initcall
maps onto, and hence does not change the init order, making the
impact of this change zero.

If CONFIG_MTD_PHYSMAP is set to m, the Cobalt mtd.ko module might get
unloaded while the drivers/mtd modules are still loaded resulting in
stale references to the destroyed platform_device instance.
Anyway, platform devices should always be registered indicated what
devices are present, _not_ what drivers have been configured.

Mark interrupts with no_action handler, cascade interrupts, low level
interrupts (bus error, halt ..) with IRQF_NO_THREAD to exclude them
from forced threading.

Because the VIA SuperIO chip only decodes 24 bits of address space but port
address space currently being configured as 32MB there is the theoretical
possibility of aliases within the I/O port address range.
The complicated solution is to reserve all address range that potencially
could cause such aliases. But with the PCI spec limiting port allocations
for devices to a maximum of 256 bytes 16MB of port address space already is
way more than one would ever expect to be used so we just reduce the port
space to 16MB.

Because Qube1 doesn't have a serial chip waiting for transmit fifo empty
takes forever, which isn't a good idea. No prom_putchar/early console
for Qube1 fixes this.

Many Makefiles in arch/mips have EXTRA_AFLAGS := $(CFLAGS) line. This
is redundant while AFLAGS contains $(cflags-y) and any options only
listed in CFLAGS (not in cflags-y) should be unnecessary for asm
sources.
