- remove unused 'irq' argument from pfm_do_interrupt_handler()
- remove pointless cast to void*
- add KERN_xxx prefix to printk()
- remove braces around singleton C statement
- in tioce_provider.c, start tioce_dma_consistent() and
tioce_error_intr_handler() function declarations in column 0
This change's main purpose is to prepare for the patchset in
jgarzik/misc-2.6.git#irq-remove, that explores removal of the
never-used 'irq' argument in each interrupt handler.

Add additional support for CPU disable on SN platforms.
Correctly setup the smp_affinity mask for I/O error IRQs.
Restrict the use of the feature to Altix 4000 and 450 systems
running with a CPU disable capable PROM, and do not allow disabling
of CPU 0.

I noticed that many source files include <linux/pci.h> while they do
not appear to need it. Here is an attempt to clean it all up.
In order to find all possibly affected files, I searched for all
files including <linux/pci.h> but without any other occurence of "pci"
or "PCI". I removed the include statement from all of these, then I
compiled an allmodconfig kernel on both i386 and x86_64 and fixed the
false positives manually.
My tests covered 66% of the affected files, so there could be false
positives remaining. Untested files are:

I would welcome test reports for these files. I am fine with removing
the untested files from the patch if the general opinion is that these
changes aren't safe. The tested part would still be nice to have.

Altix (shub2) pushes the BTE clean-up into SAL.
This patch correctly interfaces with the now implemented SAL call.
It also fixes a bug when delaying clean-up to allow busy BTEs to
complete (or error out).

Initial git repository build. I'm not bothering with the full history,
even though we have it. We can create a separate "historical" git
archive of that later if we want to, and in the meantime it's about
3.2GB when imported into git - space that would just make the early
git days unnecessarily complicated, when we don't have a lot of good
infrastructure for it.
Let it rip!
