DRA7 uses OMAP5 IO table at the moment. This is purely spurious since
the OMAP5 and DRA7 register maps are different in many aspects.

NOTE: Most of the drivers are already doing ioremap, so, there should'nt
be any functional improvement involved here, other than making the
initial iotable more accurate.

OMAP5430 is Texas Instrument's SOC based on ARM Cortex-A15 SMP
architecture. It's a dual core SOC with GIC used for interrupt
handling and with an integrated L2 cache controller.
OMAP5432 is another variant of OMAP5430, with a
memory controller supporting DDR3 and SATA.
Patch includes:
- The machine specific headers and sources updates.
- Platform header updates.
- Minimum initialisation support for serial.
- IO table init

EMIF, GMPC and DMM driver can ioremap() the address
space as part of driver intialisation and there is
no need to have static IO mapping for them.
Hence remove the un-used static IP space and let
the respective drivers manage it as part if driver
init.

Several platforms create IOMEM defines for casting to 'void __iomem *',
and other platforms are incorrectly using __io() macro for the same
purpose. This creates a common definition and removes all the platform
specific versions. Rather than try to make linux/io.h and asm/io.h
assembly safe, the assembly version of IOMEM is moved into
asm/assembler.h.

There's no need to have these defines in plat/io.h.
Note that we now need to ifdef omap_read/write calls
as they will be available for omap1 only.
While at it, clean up the includes to group them like
they typically are grouped.
