mv78xx0 and kirkwood use identical mpp code.
It should also be possible to rewrite the orion5x mpp to use this
platform code.

This patch makes the various mach dirs that use the plat-orion GPIO
code pass in GPIO-related platform info (GPIO controller base address,
secondary base IRQ number, etc) explicitly, instead of having
plat-orion get those values by including a mach dir include file --
the latter mechanism is problematic if you want to support multiple
ARM platforms in the same kernel image.

Commit 21f0ba90a447 "orion/kirkwood: reset PCIe unit on boot" made the
reset of the PCIe unit unconditional. While this may fix problems on some
targets, this also causes problems on other targets.
Saeed Bishara <saeed@marvell.com> said about the original problem: "We
couln't pinpoint the root cause of this issue, actually we failed to
reproduce that issue."
So let's restrict the reset of the PCIe unit only to the target where
the original problem was observed.

Since we're now using addruart to establish the debug mapping, we can
remove the io_pg_offst and phys_io members of struct machine_desc.
The various declarations were removed using the following script:
grep -rl MACHINE_START arch/arm | xargs \
sed -i '/MACHINE_START/,/MACHINE_END/ { /\.\(phys_io\|io_pg_offst\)/d }'
[ Initial patch was from Jeremy Kerr, example script from Russell King ]

Rather than checking the MMU status in every instance of addruart, do it
once in kernel/debug.S, and change the existing addruart macros to
return both physical and virtual addresses. The main debug code can then
select the appropriate address to use.
This will also allow us to retreive the address of a uart for the MMU
state that we're not current in.

This patch enables users to choose either the SDIO interface or UART1
(RS232/RS485). The selection can be done through kernel parameter.
By default the port would be used for SDIO interface. Passing the string
"kw_openrd_init_uart1=232" or "kw_openrd_init_uart1=485" enables either
the RS-232 or RS-485 port respectively; disabling the SDIO interface.
Anything else selects the default SDIO interface.
"kw_openrd_init_uart1=485" is ignored on OpenRD-Base as it doesn't
have RS485 port.

The support for the 2 pcie port of the 6282 has broken i/o port by switching
*_IO_PHYS_BASE and *_IO_BUS_BASE. In fact, the patches reintroduced the same
bug solved by commit 35f029e2514be209eb0e88c7d927f3bcc42a5cc2.
So, I'm adding back *_IO_BUS_BASE in resource declaration and fix definition
